Individuals who require a certificate attesting to receipt of both doses of COVID-19 vaccine can apply digitally to the Ministry of Health.
Those interested can apply online by completing the form available at: https://www.ministeriodesalud.go.cr/index.php/vigilancia-de-la-salud/certificado-de-vacunacion-covid-19
In addition to filling out the form, in order to prove identity, those who have a digital signature must complete the ballot available on the same link, sign it digitally and send it to the following email: email@example.com
Those who do not have a digital signature must send a copy of a valid identification document (identity card, passport or DIMEX) and a copy of the vaccination card received at the CCSS on both sides to the same email address.
Once the form is completed, the applicant will receive a confirmation email with a consecutive processing number. The certificate will be sent to the email address provided.
Persons requiring the vaccination certificate for international use must pick it up in person at the central offices of the Ministry of Health for the apostille process to be carried out at the Ministry of Foreign Affairs.
The delivery time for the certificate is seven days, once the requirements have been received in full. However, it is worth clarifying that the Ministry of Health receives data on inoculated persons eight days after they have received the doses of vaccines.
The certificate is issued only for complete vaccination schedules against COVID-19 administered in Costa Rica.
